  • If the applicant is under 16 years of age, this document will NOT be required.
• P0150:Documents certifying the dispatch period, status, and remuneration from the sending agency, such as a copy of a letter of dispatch from a foreign religious organization, as appropriate.(Prepared by Organization)
• P0151:Documents clarifying the outline of the sending organization and receiving organization (sect, history, name of representative, organization, facilities, number of believers, etc.) as appropriate(Prepared by Organization)
  • Please submit this document as proof of the existence of the relevant organization.
