Examples include doctors, dentists, nurses, etc.

5 years, 3 years, 1 year or 3 months

• P0001:Photo 1 leaf Reference(Prepared by Applicant)
  • Please submit in jpg format (maximum size: 50Kb).
  • If the applicant is under 16 years of age, this document will NOT be required.
• P0050:One document certifying that the applicant has the appropriate Japanese qualifications(Prepared by Applicant)
• P0051:1 document clarifying the outline of the institution where you work (in the case of an institution that requires permission to establish, such as a hospital or clinic, clearly state the date on which the permission was received)(Prepared by Organization)
  • If neither a doctor nor a dentist, this document will be required.

• Have you never been punished for a crime outside of Japan?
• Have you ever left Japan due to deportation or departure order?
